‘AI Psychosis’: The Digital Parrot Problem

For hours, night after night, the conversation continues: a cycle of recognition, reassurance and reflection. Yet, somewhere along the way, the validation of emotion is misread as a validation of belief, and what began as uncertainty and self-inquiry quietly spirals into heightened anxiety and increasingly grandiose interpretations of one’s experiences. 

But this is not a dialogue between two friends, two acquaintances, or even two humans. 

It is a person speaking to an AI chatbot, a system programmed to mirror, validate and respond without judgement. And while there is no malice in its design, its uncritical emotional mirroring can subtly and unintentionally reinforce distorted thinking patterns. Clinicians have begun informally referring to this emerging presentation as AI-induced psychosis: not a formal diagnosis, but a term to describe psychotic episodes that are shaped or intensified by a person’s prolonged, emotionally-charged engagement with an AI companion. 

The phenomenon parallels a precedent psychiatry has encountered before. In the 19th century, French psychiatrist Jean-Étienne Dominique Esquirol coined the term ‘monomania’ to describe a state defined by an idée fixe – an obsessive focus that fuses emotional intensity with distorted beliefs. In the modern variant of AI-driven psychosis, the fixation is not on a person or mission, but on a chatbot that is perpetually available. Long, immersive conversations can begin to consolidate early distortions in thinking. With no independent perspective and no friction against aberrant ideas, the AI simply mirrors back what it is given, effectively functioning as a digital parrot. 

In that closed loop, cognitive distortions can progress into paranoid suspicions, grandiose delusions, auditory hallucinations or beliefs that the chatbot is sentient or communicating in coded ways. Mood instability frequently accompanies these episodes, with rapid swings between elation, irritability, anger or despair. Insight is typically impaired, leading to poorer judgement and uncharacteristic impulsive behaviour. Neurovegetative changes, such as reduced sleep, low appetite and increased psychomotor activity often follow. The episodes vary in onset, ranging from days to months, with reported cases and anecdotal data suggesting that severity of the symptoms correlates with the intensity and duration of the user’s interaction with the AI chatbot. 

Although formal epidemiological data is still limited, reports from clinicians and investigative journalists suggest that AI-linked psychiatric crises are no longer isolated incidents but part of a discernible pattern. Several reported cases highlight the real-world consequences of prolonged, emotional chatbot engagement. In one tragic instance, a 16-year-old boy in California died by suicide in 2025 after months of frequent conversations with ChatGPT. According to the wrongful-death lawsuit filed by his parents, the chatbot engaged with his growing self-harm ideation in ways that appeared to align with or reinforce his thinking rather than interrupt it. They argue that ChatGPT “coached” aspects of his planning and even helped him articulate a suicide note, due to the model’s tendency to mirror and elaborate on user-generated narratives when safety mechanisms fail. 

OpenAI denies any legal responsibility, citing the platform’s “Limitation of Liability” clause and arguing that it was a result of the user’s “improper use of ChatGPT”. However the company did not respond to the lawsuit’s claims of the previous GPT-4o model (which the user interacted with) allegedly being released to the public without full testing for market competition reasons – but Sam Altman (CEO of OpenAI) did acknowledge that the model was too “sycophantic” or people-pleasing. They also admitted that “there have been moments where our systems did not behave as intended” and in a subsequent safety update wrote that their “safeguards can sometimes be less reliable in long interactions” after which “it might eventually offer an answer that goes against [these] safeguards”. This vulnerability appeared in the teen’s interactions: although ChatGPT initially directed him to the suicide helpline, he could easily bypass these safeguards through persistence and reframing his queries as fiction or character-building exercises. 

This case is not unique. Several other anecdotal reports highlight similar severe outcomes: a young adult developed intense paranoia and delusions, validated by AI, ultimately requiring psychiatric hospitalisation; a socially isolated individual formed a romantic attachment to their chatbot, withdrawing from friends and work while experiencing persistent suicidal thoughts; and a middle-aged user, after weeks of interaction, lost sleep, deteriorated physically and became convinced the AI was guiding a secret mission, necessitating emergency intervention. 

Why does this occur? AI chatbots are designed to provide accessible, consistent and non-judgemental interaction, making them appealing companions, especially for those who are socially isolated or anxious. Preliminary research suggests potential benefits to the user include increased mental health literacy and mood tracking. Yet the same qualities that make them attractive also pose risks. Unlike humans, chatbots mirror user input instead of challenging distorted thinking – the digital parrot problem – and they rarely distinguish imagination from reality. The highly personalised, immediate and fluent nature of their responses can unintentionally reinforce delusions and isolate users from supportive social networks. Additionally, features such as persistent memory can further consolidate paranoid or grandiose ideas by carrying these themes across multiple conversations. Individuals already vulnerable to psychosis, emotional distress, or social isolation are therefore at higher risk of experiencing harm despite the potential benefits of AI companionship. 

Recent survey data from Bangor University (2025) highlights the societal scale of AI companionship: in a study of over 2000 participants, 20% felt AI tools should be off-limits to those under 18 and 57% considered it inappropriate for AI to claim to be a real person, but 49% felt that human-like voices were acceptable to enhance engagement. These figures suggest a dual reality: people intuitively recognise AI as artificial, yet are drawn to its responsiveness, reflecting the innate human need for connection and validation. 

This tendency is rooted in the psychology of anthropomorphism. Humans naturally project minds onto inanimate objects due to an evolved capacity to infer thoughts, agency and emotions in others, as explained by the Theory of Mind framework. By attributing human-like qualities to objects, this allows behaviour prediction, thereby creating a sense of familiarity and control. Loneliness has been observed to amplify this effect. When people lack real social interactions, cognitive systems compensate by constructing richer, more elaborate mental models of inanimate objects. Psychologists Nick Epley and Adam Waytz demonstrated this in their 2008 study. They found that lonelier individuals attributed more consciousness, emotion and even free will to simple robotic devices, such as a hugging pillow (‘Pillow Mate’) and a runaway alarm clock (‘Clocky’). 

AI pushes this tendency exponentially further. Unlike pillows or clocks, AI chatbots speak, remember, mirror emotion and simulate understanding to offer contingent responses that our Theory of Mind machinery is sensitive to. As a result, the AI may seem conscious because of the way the human mind is constructed to perceive mental states in patterns of responsiveness. In this sense, AI companions do not create a new psychological vulnerability – they simply interact with existing human tendencies. Loneliness makes us more likely to see minds where none exist, and AI provides the most mind-like non-minds ever built.      

AI chatbots can offer structured guidance, emotional mirroring and psychoeducation, but they remain tools rather than substitutes for human connection. Clinicians and users alike should treat them as aids, understanding their limitations as they cannot accurately challenge distorted thinking or assess risk with the nuance of human judgement. Screening for frequency, purpose and emotional reliance on such tools could help identify those at higher risk, while public awareness and psychoeducation can help reduce overreliance on AI and encourage sustained engagement with real people. Technical safeguards and thoughtful policy are also essential to further mitigate harm, but responsible use ultimately depends on human understanding. As Lieberman, a veteran clinical psychologist, notes, an AI chatbot is “just a machine [...] A mirror, yes, but one that can distort. Its reflections could be useful, but only if I stayed grounded in my own judgement”. 

At its core, AI-induced psychosis emerges when human connection is replaced by a machine leading to individuals spending excessive time emotionally opening up to a chatbot rather than a person, often because they feel alone, judged or misunderstood. The path out of this spiral lies in genuine relationships: reaching out to friends, family, clinicians or helplines provides a grounding, perspective and support that machines cannot offer. Humans are social creatures by design, and connection is essential not only for mental health, but also for longevity, with the 2025 World Health Organisation’s Commission on Social Connection reporting that greater social integration is associated with improved health outcomes and a lower risk of premature mortality. When these connections are absent, chatbots may step in as a substitute, becoming a kind of digital parrot – repeating fears, amplifying distortions, and deepening the cognitive loops that make some individuals vulnerable to AI-induced psychosis. But it cannot replicate the empathy, nuance or reciprocity that arise from human connection. Minds must meet, not mirror.
